Immunisation Requirements

 

When enrolling a child into a child care service parents/guardians are required to provide

their child’s current immunisation status by providing Immunisation History Statement.  

The Immunisation status needs to be up to date according to the

National Immunisation Program Schedule

The Immunisation History Statement can be no more than 2 months old.

WE ARE UNABLE TO USE THE CHILD’S HEALTH BOOK AS EVIDENCE OF IMMUNISATION STATUS

A child’s current Immunisation History Statement

can be obtained by the parent/guardian through

MyGov, by logging in to their Medicare online account
Medicare Express Plus App, by logging into their Medicare account
Visiting a Medicare or Centrelink office, or
Calling 1800 653 809 to request an AIR the Statement to be posted.
